چکیده

Hypomethylating drugs are useful and have been approved for the treatment of myelodysplastic syndromes (MDS) and Chronic Myelomonocytic Leukemia (CMML). However, phase 2 and 3 studies that assessed these agents in MDS, have included o patients with CMML, and there are just a few specific reports on CMML patients. is actually authorised for the treatment of CMML patients with 10 myeloproliferative disorder, who are not eligible for haematopoietic stem cell transplantation. This hypomethylating agent in MDS is known for causing transient cytopenias, most often occurring during the first 2 cycles. Here we report a case of an atypical delayed and prol toxicity during Azacitidine treatment in a CMML patient; furthermore we also reviewed the literature regarding the efficacy of the drug and the management of hematologic adverse effects, in term of dose adjustments or alternative schedule Introduction. CMML is a clonal disorder of haematopoietic stem cell, often occurring in elderly patients, sharing heterogeneous clinical and morphological features of both MDS and chronic myeloproliferative disorders (MPD). Originally it was classified by the French-American working group as a myelodysplastic syndrome characterized by monocytosis (> 1000/microl), bone marrow (BM) monocyte infiltration, blast cells less than 5% in the peripheral blood (PB) and less than 30% in the BM. CMML has been reclassified and it is currently included in the Myelodysplastic/Myeloproliferative Neoplasms www.mjhid.org ISSN 2035-3006 nd Prolonged Hematologic Toxicity With Azacitidine and Review of Literature and Enrico Maria Pogliani – Università degli studi Milano Bicocca, Monza, Italia – Università degli studi Milano Monza, Italia. A Rare Case of Acute Myeloid Leukemia with Translocation (3:3) Presenting with Features of Chronic Myelomonocytic Leukemia

Background: Acute Myeloid Leukemia (AML) with translocation (3,3) is a form of AML that may present de novo or may arise from a previous myelodysplastic syndrome. It is often associated with normal or elevated peripheral blood platelet count and increased bone marrow megakaryocytes with associated multi lineage dysplasia. A subset of patients present with hepatosplenomegaly while a few cases h...

Arteritic anterior ischemic optic neuropathy associated with chronic myelomonocytic leukemia (cmml): a case report

Ocular involvement in patients with chronic myelomonocytic leukemia (CMML) is rarely reported, owing in part to the fact that the severe illness faced by many of these patients limits the feasibility of ocular examinations. Nevertheless, post-mortem examination of eyes from patients without ophthalmological symptoms and diagnosis of CMML has revealed ocular infiltration of atypical cells. We pr...

Meningeal Infiltration of Chronic Myelomonocytic Leukemia

Chronic Myelomonocytic Leukemia (CMML) is a hematologic malignancy considered a subtype of Myelodysplastic Syndrome (MDS)/Myeloproliferative Disease (MPD). According the World Health Organization (WHO) two subtypes of CMML, CMML-1 and CMML-2 are defined depending on the percentage of blasts in Bone Marrow (BM) and Peripheral Blood (PB). The clinical presentation is variable, but the majority of...

Chronic myelomonocytic leukemia presenting with polyserositis due to an immune-mediated monocyte activation

KEY CLINICAL MESSAGE Monocytosis is often profoundly implicated in chronic myelomonocytic leukemia (CMML). We diagnosed a 63-year-old woman with CMML involving heterochronological systemic polyserositis with infiltrating benign monocytes. The patient's pleural effusion was eliminated completely with corticosteroid therapy. We speculate that our case represents an immunological reaction with act...
